Below is a list of the important forms and links for the 2012 season. Access to several of the forms are limited to enrolled families; documents with an asterisk * require a password for access. If you are an enrolled family and do not have the password please email campnashoba@gmail.com and we will send it to you.
1. Camper 2012 Application Form.

3. *Photo Release Form. Please complete and return the photo release form.
4. Extended Day Application Form. If you require morning or afternoon extended day please complete and return the extended day application form for early morning drop-off and afternoon extended day.
5. Riders Release Form. If your child plans on taking horseback riding lessons this summer, please complete the riding release form.
6. Riding Tack Shops. A list of riding apparel stores that offer mail order service.
7. Medical Health Form. Please complete, sign and return the medical form. All campers are required to have a current history and physical form on file. There are a total of four pages.
8. Authorization to Administer Medication to a Camper Form. Please complete and return if you have any medications that your child will need to receive while at camp.
